This form is an appointment request only. Please wait to hear from a PCPLS team member to confirm the date/time.
  1. Appointment requests must be made at least 24 hours advance notice.
  2. A PCPLS team member will contact you prior to your appointment time to confirm the date/time and gather any additional information needed for the appointment.
  3. The scheduled appointment cannot exceed 1 hr.
  4. If a team member is unable to "fix" the problem, an appointment may be rescheduled in order to do more research on the topic.
  5. If you are unable to make your scheduled appointment time for any reason, please contact the library at 219-462-0524.
